Full Time Position

Position Summary

Under the direct supervision of the Director of Clinical Services, assists in the ongoing support of field clinicians and patients. Builds on the community's resources to plan and direct services to meet the needs of individuals and families within their homes and communities. Conducts required follow-up to ensure initial records meet compliance expectations. Provides clerical support to the hospice team.

Duties & Responsibilities

• Schedule weekly clinical visits with DCS and Clinical Team Leads.

• Observes, records and reports to DCS or RN Case Manager any signs and symptoms which indicate a change in the patient’s condition on demand.

• Answers telephone and calls staff as appropriate.

• Orders DME based on patient condition and RN request.

• Assists the team members in coordination of care plan with interdisciplinary team members.

• Calls all outside physicians listed on Hospice assignment (upon admission and discharge) and notifies DME company and Pharmacy.

• Calling and seeing patients as needed.

• Delivering medication and equipment to patients' homes and setting the equipment up.

Skill & Knowledge Requirements

• Competent working knowledge/skills with Windows and Microsoft Office computer software.

• Excellent organizational and communication skills (both oral and written).

• Ability to use sound judgment in decision-making and problem-solving by utilizing resource materials and communication with others.

• Ability to handle multiple tasks simultaneously.

• Ability to carry out written and oral instructions.

• Maintains a positive attitude and receives feedback and direction well.

Education & Experience Requirements

• Minimum Medical Assistant or Licensed Practical Nurse certification.

• Minimum of 1 year experience in a healthcare environment or equivalent position.

Hours

• Generally, Monday through Friday during regular business hours.

• Holidays and weekends may be required as needed.

​

Pay

• $21.00 - $24.00 per hour

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
