Position Summary
Functions as part of the Nurse Residency program cohort with a 1 year commitment to Hawaii Care Choices the Nurse Resident participates under supervision and guidance in providing skilled nursing to Hospice patients by utilizing nursing assessment, planning, implementation, and evaluation in all areas of care. The nurse resident assists in developing a plan of care for Hawaii Care Choices patients and their care givers based on the nursing process and in collaboration with the assigned mentor, physician and interdisciplinary team. The nurse resident practices and utilizes principles of palliative and Hospice philosophy to provide holistic care.
Duties & Responsibilities

1 year commitment to HiCare and the Nurse Residency program.

Commitment to complete all training, curriculum and learning modules as assigned within the program.

Practice clinical standards that complies with Hawaii Care Choices mission, vision, values, and policies and procedures.

Under supervision and with guidance completes the nursing assessment of the patient, including assessment of the care givers needs, and develops the plan of care in collaboration with the assigned mentor and interdisciplinary team that reflects skilled nursing judgment, medical necessity, measurable patient and care giver objectives, and reasonable goals.

Provides nursing services to patients and care givers in compliance with federal, state, and Hospice regulations, initiating and coordinating utilization of services that best benefit the patient and the care givers.

Documents all nursing and case management in accordance with Hospice protocol and keeps attending physicians informed of changes in patient's condition. Maintains documentation of time sheets, mileage and other records as required.

Actively participates in interdisciplinary team meetings, including updating care plans and the IDT summaries to the attending physicians.

Empowers and supports caregivers and patients through education of the disease process and prescribed care regimens.

Upholds and maintains the organization's cost containment efforts without jeopardizing the quality of hospice care.

Observes and promotes sound safety practices. Follows all policy and procedures of Hawaii Care Choices.

Provides education and training to the patients, care givers, volunteers, and other team members as deemed necessary.

Participates in meetings and committees as requested.

Participate in program development and special projects.
Skill & Knowledge Requirements

Ability to exercise independent sound judgement in planning and providing quality Hospice nursing care.

Comfortable maintaining professional boundaries.

Active listening skills frequently required.

Ability to provide health care services in private residences, along with handling and administering controlled and prescribed medications.

Ability to identify and provide for psychosocial needs of the patient and care givers.

Ability to coordinate care with the interdisciplinary team.

Requires excellent critical thinking and assessment skills.

Interest and desire to give care with a special sensitivity to the needs of the dying.

Maintenance of adequate educational and experiential level to meet the needs of the patients and care givers and regulations established by the state and federal laws.
Educational and Experience Requirements

Graduate of a registered nursing curriculum; BSN degree or higher.

Possession and maintenance of a valid Hawai'i State nursing license.

Valid CPR certification.
Hours

Primarily Monday to Friday, 8:00 a.m. to 4:30 p.m., with possibility of evening, weekend and holiday work.
Pay $40.00/hourly
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
